Before sending: Agencies may call these records CAD event reports, call-for-service logs, event chronologies, incident histories, run cards, or dispatch logs.

Hello,

This is a public records request for dispatch and call-for-service records for:

Location, call number, or incident: [IDENTIFIER]
Agency or units involved: [IF KNOWN]

Please provide the following records, to the extent maintained:

1. The complete CAD event report, event history, run card, or incident chronology.
2. Call type, priority, location, call summary, narrative notes, disposition, and closing code.
3. Every timestamp, including call receipt, dispatch, en route, arrival, transport, clear, and close times.
4. Unit assignments, responding personnel, dispatcher or call-taker identifiers, status changes, cancellations, transfers, and supervisor activity.
5. The original 911 and non-emergency call audio, radio traffic, dispatch-channel audio, and associated recordings.
6. Any address history, linked events, supplemental comments, call attachments, or related incident numbers associated with the event.
7. Any maintained export or audit history showing additions, edits, reclassification, or closure of the CAD event.

Please provide the records electronically. For tabular or database records, an Excel, CSV, or system export is preferred over screenshots or scanned printouts. For audio, please provide the maintained digital files.

If this is a daily or date-range request, a summary containing the call type, location, public narrative/summary, disposition, and event number is acceptable unless otherwise noted above.

Please redact only information that is legally exempt, produce the non-exempt remainder, and identify the specific authority for each withholding. If estimated charges will exceed $[AMOUNT], send an itemized written estimate before beginning chargeable work.
