Use this template to request police video, audio, photographs, reports, and the electronic information maintained with those files.

Before sending: Include the incident date, approximate time, location, case number, and known officer names. A narrow time window can substantially reduce review costs.

Please provide:

1. All body-worn camera recordings for every officer who responded, arrived, assisted, transported, supervised, or reviewed the incident.
2. All in-car, dash-camera, rear-seat, interview-room, holding-area, facility, drone, helicopter, or other agency video or audio associated with the incident.
3. The complete incident/offense report, arrest or probable-cause affidavit, supplements, witness statements, photographs, diagrams, evidence/property records, and related attachments.
4. The CAD event report or event history, dispatch notes, unit assignments, timestamps, and responsive 911 or non-emergency call audio.
5. A file list or index identifying each responsive recording, including the maintained filename, recording date/time, duration, assigned user or device, and evidence ID, when available.
6. The evidence audit trail, access history, upload history, sharing history, redaction history, deletion or retention events, and other maintained metadata for the responsive digital evidence.

Please preserve the original files while this request is pending. Produce electronic records in their existing or native format when available, with the original audio track and maintained metadata. Please do not substitute a screen recording or lower-quality conversion if the maintained file can be exported.

If a recording is withheld or redacted, please identify the specific legal authority for each withholding and produce all reasonably segregable non-exempt portions. If estimated charges will exceed $[AMOUNT], please provide an itemized written estimate before beginning chargeable work.

Rolling production is welcome. Please confirm receipt and provide the tracking number.
