Use this request for charging affidavits, incident reports, booking records, jail records, probable-cause documents, and connected evidence.

Before sending: Provide the full name and approximate arrest date when possible. Add a case number, booking number, date of birth, or agency to distinguish people with similar names.

Please provide, to the extent maintained:

1. The arrest affidavit, charging affidavit, probable-cause affidavit, incident/offense report, and every supplemental report.
2. The booking sheet, booking photograph, intake record, arrest card, fingerprint card, property/inventory receipt, and bond or release information.
3. The arrest and detention chronology, housing or movement log, release record, and public jail register entries.
4. Warrants, returns, citations, notices to appear, charging documents, and publicly available court-transfer records maintained by the agency.
5. Photographs, audio, video, witness statements, diagrams, evidence records, and attachments associated with the arrest or incident.
6. The CAD event history, dispatch notes, unit assignments, and related call numbers.
7. Any corrected, amended, or supplemental version of a responsive report, together with the maintained date of each version.

Please provide electronic copies. If an item is available through a separate clerk, jail, court, or agency custodian, please forward the request or identify the correct custodian and record location.

If any portion is withheld, please redact only the exempt information, produce the remaining portions, and identify the specific legal authority for each withholding. If estimated charges will exceed $[AMOUNT], provide an itemized written estimate before beginning chargeable work.
