PALM COAST, Fla. — A Daytona Beach Shores EMT and public-safety employee accidentally shot himself in the leg while handling a handgun at a Palm Coast residence on July 12, 2026, according to a redacted Flagler County Sheriff’s Office report.
The report identifies the injured man as John Wallace Frazier IV. It also identifies Billie Richard Phillips III, Frazier’s father-in-law, as a Daytona Beach Shores Fire Department lieutenant with medical training.
What the report says
Frazier told deputies he retrieved a Glock 22 from the glove box of a vehicle parked in the driveway at approximately 7:22 p.m. He said he removed the magazine and intended to clear the pistol, but forgot to remove the round from the chamber before pulling the trigger.
The discharged round entered just above Frazier’s left knee and exited near his left shin, the report states. Frazier then called Phillips, who secured the firearm, applied direct pressure to the wound and helped him into a vehicle.
Flagler County Rescue 41 treated Frazier at the scene. The report says Frazier declined ambulance transportation and was driven to Halifax Hospital by Phillips.
Deputies reported obtaining video from a Ring security camera. According to the narrative, the recording showed Frazier sitting near the open driver-side door, captured the sound of one gunshot and then recorded a male voice saying to call Billie.
Investigators found no evidence that the shooting was intentional. Deputies also reported finding no bullet holes in nearby vehicles, homes or other property. The firearm, vehicle and available video evidence were documented, and the case was classified as non-criminal.
Daytona Beach Shores connection
Daytona Beach Shores’ official 2026 Public Safety Awards material identifies John Frazier as a public-safety employee and Bill Phillips as a lieutenant. Both were recognized as part of a team credited with a lifesaving response to an unrelated 2025 cardiac-arrest call.
No arrest or criminal charge is listed in the Flagler County report concerning this shooting.
